The first and only certified specialist in male pelvic floor rehabilitation in Saudi Arabia and the Gulf

Sultan Al Mefleh

Senior Physical Therapist - Male Pelvic Floor Rehabilitation

With over fifteen years of experience in physical therapy and rehabilitation, I believe the patient is the foundation of every successful treatment plan. Exceptional care begins with listening to the patient's full concern and involving them in setting goals and treatment plans that suit them and their condition. A complete plan is one that considers the patient's physical state, its impact on their mental well-being, and their overall quality of life.

About Me

A physical therapy specialist in the rehabilitation and treatment of men’s pelvic floor muscles. I received my training in the United States of America, and I was honored to bring this specialty to my homeland, the Kingdom of Saudi Arabia. Pelvic health rehabilitation, to me, is not just a “job" It is the passion and ambition through which I strive to improve the quality of life of men who suffer in silence, moving them toward a better life.

Conditions I treat

Pelvic floor physical therapy specializations for men

Urinary & prostate disorders
01

Urinary & prostate disorders

Prostate conditions cause bothersome urinary symptoms such as frequency and difficulty urinating, as well as pelvic and lower-back pain. This greatly affects the patient's psychological, social and daily quality of life. A pelvic floor physical therapist helps rehabilitate the affected muscles so urination returns to normal.

Colon & constipation disorders
02

Colon & constipation disorders

Colon disorders and constipation are the main causes of rectal prolapse and hemorrhoids. Patients often struggle with bowel movements and spend long periods in the bathroom with little result. A physical therapist teaches you how to use your abdominal muscles and breathing while relaxing the anal muscles to improve evacuation.

Coccyx pain
03

Coccyx pain

Coccyx pain is one of the most annoying complaints, whether caused by a fall and direct injury to the area or by prolonged sitting. A physical therapist treats the coccygeal vertebrae, increases their mobility and addresses the connected muscles to help eliminate the pain completely.

Chronic pelvic pain
04

Chronic pelvic pain

If you suffer from pain in the pelvic region due to an injury, surgery or any other cause, visit a physical therapist to examine the pelvis, joints, ligaments and surrounding muscles, identify the source of the pain and work on treating it.

Men's sexual health
05

Men's sexual health

Physical therapy can help treat genital pain, premature ejaculation and erectile dysfunction caused by weakness or spasms of the pelvic floor muscles. Treatment includes manual therapy for these muscles, pelvic floor strengthening exercises, and modalities such as electrical stimulation, shockwave and electromagnetic therapy.

Frequently asked questions

Answers to what clients ask most

The initial session (consultation) is the most important session in the treatment plan. I will listen carefully to the health problem you came for, then ask you some questions related to your condition about all pelvic functions ( urinary, bowel, and sexual ) as well as your lifestyle, physical activity, and healthy and unhealthy habits. Then, together, we will draw a timeline of how your symptoms and health problem developed, which will help you understand the root cause of your condition and know how to treat the main cause rather than just relieving the symptoms. After that, I will explain the examination method and the full expected treatment plan, and then perform a clinical examination of the muscles related to the pelvis and pelvic floor muscles to confirm the diagnosis.

The consultation session does not require any special preparation other than bringing as much information as possible about the health problem you are experiencing, and any previous test or imaging reports related to your condition. I recommend writing down your symptoms and questions on paper or your phone so that nothing you wish to share or ask me about during the session is forgotten.

At Pelvic Health Clinic, we will handle your information and the details of your complaint with the utmost privacy and complete confidentiality. We respect your privacy and the sanctity of your body. I understand how difficult it can be to share some sensitive topics related to your health condition, and I will help you discuss these sensitive matters with great respect, professionalism, and care. You have full freedom to share what you wish to share and to keep private what you wish to keep private. I always make sure to obtain your permission before taking any step in the examination or treatment. And the examination and treatment will always be performed while covering the private parts.

Some pain during the treatment session is expected, and it is one of the main reasons the problem and muscle tension exist. However, as a therapist, I must keep the pain level tolerable for the patient throughout the session while treating the painful areas. It is not logical to expect the patient to relax, or the targeted muscles to relax, while the patient is feeling unbearable pain.

Yes, and significantly. Pelvic floor muscle problems and disorders are well-known, recognized, and medically proven conditions. However, in the Arab world and especially among men, people suffering from these disorders often feel embarrassed to talk about them or share their full experience with those around them, as they believe it may somehow harm their image or masculinity. Because of this, most patients think they are the only ones in their circle going through this struggle — and they endure it in silence.

Yes, absolutely, by the grace of God. I understand that my answer (as a service provider) may be met with some doubt. However, you can search for people I have previously served and read about their experiences and impressions. The equation is simple, after God's guidance: Correct diagnosis + comprehensive, competent care with a pelvic floor specialist + the patient's commitment to instructions and exercises = full recovery, God willing.

The expected treatment duration varies depending on the medical condition, its severity, its cause, and how long the patient has been suffering from these disorders. However, in most cases, the treatment plans are as follows: 4 to 6 sessions ( twice per week ) for most mild and moderate cases. 6 to 8 sessions ( twice per week ) for advanced cases, such as severe pelvic floor muscle spasms, chronic constipation, or a previous medical history involving hemorrhoids, fissures, or anal surgeries.

You can start by booking an appointment at the virtual clinic (remote consultation), where I will listen to your full medical history and ask you about all functions and symptoms related to your health condition. Based on that, I will provide you with a detailed explanation of the problem and the appropriate treatment approach, whether through home exercises, behavioral advice that supports your recovery, or by guiding you to the right physician close to you ( if available ) for the required medical tests. If your condition requires pelvic floor physical therapy sessions, I will coordinate a treatment plan that suits you for coming to Riyadh and completing the treatment plan in the shortest possible time during your stay.

You can book an appointment at any time without needing a direct referral from your treating physician. However, if there have been no previous medical visits or examinations, I will take your medical history, perform a pelvic floor muscle assessment, and help you understand and diagnose your condition. Then I will refer you to the appropriate physician for the medical tests related to your case, so that no medical cause preventing your full recovery is overlooked, God willing.
